Nurse Practitioner (Wound Care)

Job Summary:

The Nurse Practitioner (Wound Care) at Smart Choice Medical Services is critical in fulfilling the promise of delivering exceptional wound care services in the comfort of patients' homes, in clinic, and long-term care facilities. This position is responsible for providing wound management working collaboratively with the medical director or attending physician; creating and maintaining a plan that results in successful wound care outcomes; and when needed will provide consultative phone calls and/or telehealth visits with home health agencies or nursing staff. This opportunity will require frequent travel.

Duties and Responsibilities:

1. Upon hire you will receive wound care training and expected to perform the following duties related to up-to-date topical wound care products and therapies, including:

a. Developing and managing an individualized wound care treatment plan for optimal healing outcomes or chronic maintenance.

b. Ordering, applying, and managing of the products on wounds, also including negative pressure wound therapy.

c. Providing hands on wound care to patients including debridement of wounds by enzymatic, sharp, or autolytic methods when appropriate.

d. Ordering, reviewing, interpreting, and integrating diagnostic studies and laboratory findings for initiating or modifying the treatment plan.

e. Collaborating with the patient’s primary care provider when comorbidities are contributing to or affecting the healing of a wound to determine a comprehensive plan.

f. Updating the primary care provider on the status and management plan of the wound as appropriate.

g. Prescribing medications and/or durable medical equipment to assist in healing of wounds.

h. Providing the required billing information for services provided and maintains the documentation necessary for those services billed.

i. Reviewing patients’ prior medical records and maintains their current medical record related to the wound diagnosis and contributing comorbidities.

j. Ensuring Patient chart encounters will be closed according to the standard of care.

k. Assist with the identification of wound care quality measures and provide data and documentation for tracking of quality measures.

l. When necessary, educating referral facility leadership and staff with appropriate identification of wound types, prevention of wounds, and nursing care of wounds.

m. Instructing patients and families regarding wound treatments and management plan for optimal wound outcomes.

n. Triages telephone calls and provides consultation.

2. Participates in professional development activities and maintains professional affiliations.

3. Completes required number of continuing medical education (CME) hours for license and certification.

4. Travels throughout the assigned area to patients’ homes.

5. Attends required meetings.

6. Performs other duties as assigned.

Minimum Qualifications:

· Graduate of an accredited Nurse Practitioner Program.

· Current, unrestricted nurse practitioner license the state of Mississippi.

· Certification by a recognized national professional accrediting organization and in good standing.

· DEA License

· BLS and CPR certified

· Ability to pass comprehensive background checks.

· Excellent clinical and communication skills.

· Ability to interpret clearly and establish/maintain effective working relationships with patients, medical staff, and facilities.

· Ability to travel for training withing 30 days of hire,

· Ability to work independently and travel extensively throughout the assigned area.

· Valid Driver’s License

Preferred Qualifications:

· Wound care experience

· Previous Nursing Home experience or exposure

Physical Requirements:

· Able to lift 100 pounds to assist fallen residents.

· Able to lift 10 pounds for routine performance of essential functions.

· May be exposed to adverse indoor and outdoor environmental conditions, bloodborne pathogens, physical hazards (e.g., needle pricks, etc.) and chemical hazards.

Job Type: Full-time

Pay: $50.00 - $65.00 per hour
